Form SSA-454-BK is used to collect data about medical and non-medical sources, updated work status, educational/vocational training, and current activities of daily living.
It is designed to collect information during a continuing disability review (CDR). It asks about the current status of your condition since the date of the last favorable medical disability decision. The last decision becomes what Social Security calls the "Comparison Point Decision" (CPD).
The form is also used to collect information if you apply for Expedited Reinstatement (EXR) of disability benefits.
