As you have previously been made aware, I am the Health Care Proxy for Jane Doe pursuant to her Health Care Power Of Attorney dated XXXXXX XX, XXXX, of which you have a copy.

Jane is in the condition described in said advance directive.

Pursuant to the subject document, I instructed you on XXXXX XX, XXXX to take her off the ventilator.. To date, you have not complied with my request.

You are hereby notified that neither Jane or her estate are liable for any and all medical and other expenses from the date of my request, except for those expenses which would be necessary to carry out my specific demands.

It is my continued hope that you will comply with my demands immediately. If not, I will seek redress by any and all available legal means, including seeking damages.
